Your Data as a Client
Taylored To You Clinic & Consultation is committed to protecting client health information. We create a record of the care and services you receive from us. This record is needed to provide you with quality care and to comply with certain legal requirements. All records are stored securely utilizing a HIPAA-compliant electronic health record system (Session Health). Information Security is further prioritized by following cyber security industry standards per consistent consultation with a cyber security professional. We practice all this with transparency and are open to further inquiry about security practices from current, past, and potential clients. This might look like a deeper dive into steps taken to secure data or requests for informed consent documents prior to deciding if you want to begin therapy services with us. In Texas, Licensed Professional Counselors & Associates must keep client records for a minimum of seven years after the last service date, or, in the case of working with minors, five years after the minor reaches the age of majority (18).
We share info with third parties primarily through the client’s written consent using a specific Release of Information (ROI) form, detailing what, who, and why, but we can also share without consent in specific legal exceptions like imminent harm, suspected child abuse, or court orders. We will always aim to share only the minimum necessary information.

Requesting Health Care Records
Current and former clients have a right to request copies of their records maintained by Taylored To You Clinic & Consultation PLLC. Requests must be submitted using the Records Request Form available to download below:

To help prevent delays, please include the following information with your request:
• Client full legal name and date of birth
• Current phone number and or email address
• Description of records requested including specific dates if applicable
• Preferred method of delivery such as secure electronic delivery, mail, or in person pickup
• Client signature and date
If records are requested on behalf of another individual, documentation verifying legal authority is required. Examples include guardianship documentation or medical power of attorney.
Identity verification may be required to protect client privacy and confidentiality in accordance with applicable state and federal laws.

Clients or members of the public who wish to file a consumer complaint may do so through the Texas Office of the Attorney General Consumer Protection Division.
